NOTICE AND SCHEDULING

Carrier Notice:  DD Forms 1840/1840R are accepted on a walk-in basis during regular hours of operation.   Claimants must submit their DD Form 1840R to a military claims office within 75 days of delivery.   See Carrier Notice – Information and Carrier Notice – Instructions for additional guidance.   

Claims Submissions:  Claims are also accepted on a walk-in basis during regular hours of operation.   If you wish to schedule an appointment to submit a claim or inspection, please call (703) 805-4380/4395.   

Medical Affirmative Claims: Personnel who desire to discuss claims  against third parties for the value of medical care provided by military treatment facilities or TRICARE should call (703) 805-4387/4384.
