CLAIMS ARISING FROM MILITARY ACTIVITIES

If you intend to file a tort claim against the Army, you should contact the Army claims office at the installation where the loss, damage or injury  occurred.

If a claim arises from an incident not on a military installation, investigative responsibility is assigned according to the area where the incident occurred.   For example, Fort Belvoir is assigned responsibility for claims incidents in Prince William and Loudoun Counties; Fort Myer is assigned responsibility for off post incidents in Fairfax and Arlington Counties.

Claims are filed on a Standard Form 95 (Claim for Damage, Injury or Death).  A copy of which you can fill online  and then print is at www.gsa.gov at “Forms Library.”  Read the instructions on page 2 of the form before filling it out.
