PASSPORTS AND VISAS
The passport agent within Personnel Operation Branch (POB) will assist Soldiers and Family members in obtaining dependent, official, and diplomatic passports. In addition to the normal passport application (DS-11 for new applicants and DS-82 for renewal), photos, and primary evidence of citizenship required, Soldiers and their Family members will need authorization from their sponsoring agency. Also, a completed DD Form 1056 is needed to apply for a no-fee passport.
NOTE: Soldiers requiring a passport must make an appointment with PSOB first.

SERVICE MEMBERS
- ID card and orders only are required for most locations such as: Germany, Italy, and Korea. No passport will be issued unless required by Foreign Clearance Guide (ie. Embassy duty).
- PCS orders must state “Official” or “Diplomatic” passport required for service member if overseas command requires it (see HRC Branch Manager and Levy Section if orders need amending).
DOD CIVILIANS
- CAC Card
- Travel orders or Command Sponsorship Letter on Command’s Letterhead explaining purpose of travel and required need for passport and/or Visa.
- Contractors – contract showing expiration of company’s contract and letter of purpose needed from Contract Agency (limited passport is issued based on contract time).
FAMILY MEMBERS
- Passports are required for all overseas locations.
- Family members must be listed on orders if traveling with sponsor and have valid military ID Card.
- No Fee Passports must be obtained prior to receiving command sponsorship.
- Sponsor must be present with all family members applying for passports.
- Due to child protection laws, children must be physically present with both parents when applying.
- The State Department will not expedite Family member applications. Please apply as soon as you receive orders.
FORM DS-11 For new applicants, all children 16 and under, and passports issued over 15 years ago
or
FORM DS-82 For Renewal (Answer the following list of questions. If you answer “NO” to any of them, fill out form DS-11 instead)
- I can submit my most recent passport AND
- I was at least 16 years old when my most recent passport was issued AND
- I was issued my most recent passport in the last 15 years AND
- I use the same name as on my most recent passport or since my passport was issued, I have had my name changed by marriage or court order and can submit proper documentation to reflect my name-change.
NOTE: THE DS-11 AND DS-82 MUST BE TYPED ONLINE TO OBTAIN 2D BARCODE ON TOP LEFT AND ONE ON LEFT SIDE OF FIRST PAGE OF APPLICATION. HANDWRITTEN APPLICATIONS ARE NO LONGER ACCEPTED FOR DOD PASSPORTS. DO NOT SIGN APPLICATIONS UNTIL AT PASSPORT OFFICE.

- PCS/TDY Orders
- Original state issued Birth Certificate -- long form w/raised seal. Hospital and Birth Abstracts are not accepted (with DS -11)
- Original Marriage Certificate (with DS-11 or DS-82 w/name change)
- If applying for stepchildren 16 and under (FORM DS-3053 – must be signed/notarized by biological parent if not physically present w/copy of their driver’s license)
- Previously issued passport (DS-82)
- Visa Application if required (see link on back of pamphlet)
- 4 Passport photos, size 2” x 2”, w/white or off-white background (do not wear uniform or anything with a logo or writing)
