DEERS Updates

Primary ID must be a photo ID.

Initial Enrollment:

    • New Spouse
  • Sponsor - Military Member or Retiree must be present or customer must      present a valid Power of Attorney (POA), special or general.
  • Marriage Certificate
  • Social Security Card
  • Sponsor’s Divorce Decree (If Applicable)
  • Two (2) Valid Forms of Identification
  • NOTE: All NEW spouses can be entered into DEERS with the original      marriage certificate and a photocopy of two valid form of IDs (only if they      are not present). When being issued an ID card for the first time, the new      spouse will have to provide two original forms of identification.
    • Newborns
  • Sponsor - Military Member or Retiree must be present or customer must      present a valid Power of Attorney (POA), special or general.
  • Birth Certificate/Live Birth Documentation
  • Social Security Card
  • NOTE: If the spouse is entering the child or children into the system, he/she      will need to be in DEERS under the sponsor and have a general power of      attorney.
    • Stepchildren
  • Sponsor - Military Member or Retiree must be present or customer must      present a valid Power of Attorney (POA), special or general.
  • Marriage Certificate
  • Birth Certificate
  • Social Security Card
  • Two (2) Valid Forms of Identification (18 yrs. & older)
  • NOTE: If the spouse is entering the child or children into the system, he/she      will need to be in the DEERS under the sponsor and have a general power of      attorney.
    • Wards & Foster Children
  • Sponsor - Military Member or Retiree must be present or customer must      present a valid Power of Attorney (POA), special or general.
  • Court Documentation (Legal and Physical Custody)
  • Birth Certificate
  • Social Security Card
  • Two (2) Valid Forms of Identification (18 yrs. & older)
  • NOTE: If the spouse is entering the child or children into the system, he/she      will need to be in the DEERS under the sponsor and have a general power of      attorney.
Renewal of CACs:
    • Geneva Convention Identification Card & Civilian CAC
  • Update CAC 30 days prior to expiration
  • Two (2) Valid Forms of Identification
    • Contractor’s CAC & Other Federal Affiliate’s CAC
  • Updated information in CVS with a new expiration date.
  • Two (2) Valid Forms of Identification
  • NOTE: Contact POC to ensure that information has been updated in CVS.

Renewal of Dependent ID Cards:

An online, fillable DD 1172 form may be accessed by clicking HERE.

    • Spouse
  • Sponsor - Military member or Retiree must be present; OR customer must      present a Special orGeneral Power of Attorney; OR a DD Form 1172 (see      note)
  • Two (2) Valid Forms of Identification
  • NOTE: The DD Form 1172 should be generated from a DEERS office (which      can be faxed from ID office to ID office) or filled out, signed, dated and      notarized by the sponsor.
    • Children (10yrs. – 20yrs.)
  • Sponsor - Military member or Retiree must be present; OR customer must      present a Special orGeneral Power of Attorney; OR a DD Form 1172 (see      note)
  • NOTE: The DD Form 1172 should be generated from a DEERS office (which      can be faxed from ID office to ID office) or filled out, signed, dated and      notarized by the sponsor.
    •Sponsor - Military member or Retiree must be present; OR customer must      present a Special orGeneral Power of Attorney; OR a DD Form 1172 (see      note)
  • School Letter (The letter should state that the individual is a full time student      and his/her expected graduation date.)
  • Two (2) Valid Forms of Identification
  • NOTE: The DD Form 1172 should be generated from a DEERS office (which      can be faxed from ID office to ID office) or filled out, signed, dated and      notarized by the sponsor. The DD Form 1172 must state the sponsor      is providing over 50% of the annual support for the dependent      child/children.

Replacement Identification Card:

    • Lost or Stolen cards
  • Sponsor - Military member or Retiree must be present; OR customer must      present a Special orGeneral Power of Attorney; OR a DD Form 1172 (see      note)
  • Two (2) Valid Forms of Identification

  • NOTE: The DD Form 1172 should be generated from a DEERS office (which      can be faxed from ID office to ID office) or filled out, signed, dated and      notarized by the sponsor.
    • Reenlistments or Promotions
  • Current Orders
  • Two (2) Valid Forms of Identification

Geneva Convention (GC):

    • Senior Executive Service (SES) member
  • Travel orders or memorandum from organization
  • Two (2) Valid Forms of Identification
    • Civilians
  • Travel orders or memorandum from organization
  • Two (2) Valid Forms of Identification
    • Contractors
  • Entered into DEERS through CVS
  • LOA with travel dates
  • Two (2) Valid Forms of Identification
  • NOTE: Contractors are eligible for one of the following forms of CACs      dependent on deployed status:
       * GC CAC – LOA stating “Mission Essential”
       * DoD IP CAC – LOA with travel dates of 365 days or more
       * DoD ID CAC – LOA with travel dates for less than 365 days
    • Other Federal Affiliates {Department of State (DOS) and Federal Bureau of Investigation (FBI)}
  • Entered into DEERS through CVS
  • LOA
  • Two (2) Valid Forms of Identification
Other Transactions:
    • Divorced Sponsor - Army Personnel ONLY
  • Statement of Service or DD 214 or Dates of inclusive service from the      servicing personnel
  • Final Divorce Decree
    • Divorcees (Dependents) - Army Personnel ONLY
  • Social Security Card
  • Current Address
  • Driver’s License
  • Marriage Certificate - Sworn Statement
  • Sponsor’s DD 214 / Statement of Service
  • Final Divorce Decree
  • Two (2) Valid Forms of Identification
